OUR HOURS: Monday & Wednesday: 8am - 5pm | Tuesday & Thursday: 9am - 6pm | Friday: Closed
For your convenience, we've put new patient forms on our website. All forms are interactive/fillable to make it easier for you to complete. You can either complete and print at home or save and email to us.
If emailing, simply save the form and send to info@redmondteeth.com We suggest naming the saved file with your name and saving it to your desktop to make it easy to find. From there, you can simply attach and send to us.
